We offer the Illinois evaluation and application process online and the fee is $575. You will receive an evaluation and completion of the Illinois Reinstatement Packet, including the 3 peer interviews. We are experienced with this application, so you are assured to get the form completed correctly and successfully. This application process is lengthy and time consuming which is why the fee is more than a standard evaluation.

Step 3: Complete the documents in your client portal then book your appointment. A standard appointment is available at no additional charge. If you need a rush appointment, we offer a standard rush that will get you an appointment sooner if needed.
