RETURN TO DUTY PROGRAM:
The Affordable Evaluations Return to Duty Program fee you paid includes your initial evaluation, follow-up consultation, certificate of completion, and completion and submission of your final report. (For CDL holders, this fee also includes updating your Clearinghouse account through step five.) Your RTD report will be sent to the required agency or company. If you have more than one outstanding violation that will require an additional report, this will be billed to you in the amount of $100 per additional report. Any services provided outside this scope may be billed accordingly or will be due to the agency who renders additional services. It is YOUR responsibility to know for certain that you have an outstanding violation that requires this program and whether you have a lockout or suspension date that cannot be changed. Once services are rendered, a refund will not be approved.
COURT/LEGAL ASSESSMENTS:
The assessment type you selected and paid for will cover exactly what that package includes. If you chose a standard evaluation, but later you are required to have a more comprehensive evaluation, additional fees will be incurred. Other more comprehensive assessments are available at an additional charge. It is YOUR responsibility to know whether the agency requiring you to submit to an assessment will accept an evaluation report from our office with our credentials and whether online services are acceptable. Once service is rendered, a refund will not be approved. Your evaluator is specifically licensed in several states and nationally certified for all 50 states. However, each case manager is different so you are encouraged to do your due diligence prior to paying for services.
DRIVER’S LICENSE REINSTATEMENT APPLICATIONS:
This service includes your substance abuse evaluation report and the completion of the evaluator’s portion of the state application only. This fee does not cover the management of your file and submission. If your application process requires any additional documents, such as proof of residency, etc., this is YOUR responsibility. Should your application be denied or rejected by the state due to an error on our part, we will absolutely complete a second application at no charge. However, if your application is denied or rejected due to an error on your part, it will be your responsibility to pay for additional time required to complete your case.
DRUG ALCOHOL TESTING:
Affordable Evaluations does not provide or require testing. However, on some occasions, you may be asked to submit to a test with a DOT certified laboratory for the completion of your case, which would be an additional fee to the lab.
CDL DRIVERS:
Once you complete the RTD program with us, you will be required to submit to a Return to Duty drug alcohol screen prior to being reinstated to drive. You will then be required to submit to random follow-up testing for, at minimum, the next 12 months of employment. ALL testing and recording of the testing, will be between you, your employer, and the laboratory. DOT does NOT give SAPs the ability to send you for an official test nor does a SAP have the ability to record the results of your testing into your Clearinghouse account. While most companies will pay for your testing, some will not. If this is the case, please prepare for additional expenses to be paid to your employer or to whomever you choose to manage your testing program.
PAYMENT OF SERVICES:
Affordable Evaluations has a NON-REFUNDABLE policy. Refunds are given ONLY by management approval on a case-by-case review. If a refund is requested, then approved, you will only receive a PARTIAL refund as administrative fees will be assessed to cover time spend with your case and card processing fees that we pay at the time you paid for services that are non-refundable to Affordable Evaluations. If granted, the amount of the partial refund will be decided by management on a case-by-case basis.
APPOINTMENT NO SHOW:
No Show: If the client fails to attend a scheduled appointment without providing prior notice or a valid reason, the client shall be considered a “no show.” In such cases, the client will be liable to pay a $50 dollar no show fee or forfeit any deposit made to hold your appointment. You may NOT be driving when you check-in to your virtual call. If you are driving at this time, the call will be terminated, and you will be charged a no-show fee.
PAYMENT DISPUTES:
I understand that if I, or the person who paid on my behalf, disputes the payment made for services, my ENTIRE file will be shared with the bank during the dispute process. Therefore, I give Affordable Evaluations permission to release my records in their entirety, including HIPPA protected files, to the bank and or card processors in which a dispute is filed. All banks and or card payment processors will be given ALL my records established with Affordable Evaluations in the dispute process.
THEFT OF SERVICES:
If you receive services then do not pay or take back your payment (dispute the charge), or pay with an unauthorized payment method, this is called THEFT OF SERVICES and is punishable by law.
You can be criminally charged with theft of service if, with the intent to avoid payment, you obtain a service by means of threat or deception, or unauthorized use of payment method. Theft of service charges can also arise if you agree to make payment in exchange for a service, and then refuse to pay for the service once it’s rendered. If you knowingly or intentionally secured a service by agreeing to pay, then failed to pay the service provider after the service was rendered; or disputed the charges with your bank with the intent in getting a refund and therefore receiving the service for free, this is theft of services.
Please note that Affordable Evaluations WILL file charges with your local authority should this occur.
I have read and understand the above information and agreements.
I understand that if I or the person who paid on my behalf disputes the payment made, ALL my secured files will be released to the bank and or card processor for dispute response purposes.
I understand that if a dispute is filed and Affordable Evaluations loses the dispute because of fraudulence or intent to not pay on my or my payor’s part, theft of services will be filed against me to my local law authorities.
Responsibility:
Court/Legal Evaluation: It is your responsibility to make sure this service will be approved by the agency requiring you to get the evaluation.
Return to Duty Program: It is your responsibility to make sure you have an open violation which requires you to take the return to duty program. It is also your responsibility to find out if you have a suspension date they may impact your return to duty eligibility.
Refund Schedule:
Once payment is made and nothing else has been completed, you may receive a full refund less a $25 dollar administrative fee.
Once payment is made, your documents have been signed, you may receive a refund less a $50 dollar administrative fee.
Once payment is made, documents have been signed, and you have reserved an appointment on our calendar, you may receive a refund less a $75 dollar administrative fee.
Once payment is made, documents have been signed, an appointment has been reserved on our calendar, and the video call has been completed, you will NOT be entitled to a refund of any amount.
By paying for your service, you are agreeing to the Affordable Evaluations refund policy.
